Important Privacy & Reporting Information – Employee Assistance Program (EAP)
As part of Clear Health Psychology's EAP service, all employees are required to complete a brief, non-identifying EAP feedback and outcomes form following each EAP appointment.
This form does not collect information that identifies you personally and is designed solely to assist Clear Health Psychology in monitoring service utilisation, identifying wellbeing trends, and evaluating the effectiveness of our EAP program.
All information provided is treated as strictly confidential and managed in accordance with the Australian Privacy Principles, the Privacy Act 1988 (Cth), and the professional standards and ethical guidelines of the Australian Health Practitioner Regulation Agency (AHPRA).
Your responses will only be accessible to authorised members of the Clear Health Corporate Services team and will not be shared with your employer in an identifiable format.
Information collected through these forms may be aggregated and incorporated into de-identified reporting provided to your employer. These reports contain trend and utilisation data only and cannot be used to identify any individual employee.
Your clinical records, appointment details, discussions with your clinician, and personal information remain confidential and will not be disclosed to your employer without your explicit consent, except where required by law.
